Fibromyalgia: making sense of persistent widespread pain

Fibromyalgia is a real, long-term pain condition. It causes pain in several areas of the body, often alongside fatigue and unrefreshing sleep. A normal X-ray does not make these symptoms any less significant. The aim is a clear explanation and a workable treatment plan.

What does fibromyalgia mean?

Changes in how the nervous system processes pain play a role in fibromyalgia. Fibromyalgia itself is not an inflammatory joint or muscle disease. The symptoms are not imagined or automatically explained by a mental health condition.

Sleep difficulties, severe tiredness and problems concentrating may accompany pain. Stress can affect symptoms, while pain can also affect daily life and mood. Other conditions may be present at the same time.

How is it diagnosed?

Diagnosis relies on discussing the distribution and duration of symptoms, associated problems and a physical examination. No single blood test or scan can confirm fibromyalgia.

Investigations are chosen to look for other or additional causes. Previous reports, a medication list and a brief account of treatments already tried are helpful. Normal findings alone are not enough to attribute all symptoms to fibromyalgia.

How can treatment help?

Treatment usually combines several elements: clear information, individually adjusted movement, approaches to fatigue and sleep problems and, when appropriate, psychological support for living with pain. Such support does not mean the pain is “just psychological”.

Medicines may be considered for particular symptoms, with benefits and side effects reviewed. A cure or complete freedom from pain cannot be promised. Small, specific goals help assess progress. Care may involve primary care, pain medicine, rheumatology and other support. Access to additional care needs to be coordinated.

What matters even after diagnosis?

New symptoms should not automatically be attributed to fibromyalgia. Marked joint swelling, fever or newly feeling unwell needs medical assessment.

Frequently asked questions

Do normal blood tests mean nothing is wrong?
No. They may make certain other causes less likely, but do not disprove your pain or fibromyalgia.
Must I keep increasing exercise despite symptoms?
No. Activity is adjusted to your capacity and response. A rigid progression plan or ignoring marked deterioration is not an appropriate general approach.
Can OrthoCoast provide all of my treatment?
Orthopaedic assessment focuses on musculoskeletal symptoms. Comprehensive fibromyalgia care may require other specialties; responsibilities and next steps need to be clarified separately.
