Treatment

Evocell as an additional treatment for back symptoms

With Evocell, you lie on a treatment couch that transfers rhythmic mechanical impulses to your body. We discuss whether it could usefully complement your treatment plan.

Understanding

A closer look

Your back feels tight and stiff. Straightening up after sitting for a long time is difficult, and painful muscle knots make movement uncomfortable. In this situation, it may be appropriate to consider a supportive physical treatment alongside assessment and active treatment. OrthoCoast has an Evocell mechanotransduction couch for this purpose. It transfers rhythmic mechanical impulses to the body. Appropriate treatment aims to support muscle relaxation and relieve symptoms. During your medical assessment, we discuss whether this fits your findings and treatment plan.

What happens during treatment

You lie on a dedicated couch. Mechanical impulses are transmitted through its surface to your body. Settings and positioning depend on the planned treatment and how you tolerate it. No injection is needed. The technical term mechanotransduction describes how cells convert mechanical stimuli into biological signals. This process explains a mechanism, but does not in itself establish the benefit of a particular treatment. What matters for your decision is whether the treatment could be a useful addition for your symptoms and what you can expect from it.

Further information

Back symptoms for which Evocell may be considered

The treatment may be considered as a supportive component for selected back symptoms, for example when marked muscle tension or painful areas of muscle tightness, known as myogeloses, are involved. Associated muscle tension may also play a part in symptoms around the small spinal joints or the sacroiliac joint. A diagnosis name alone is not a recommendation for this method. Examination comes before treatment. Where needed, we coordinate the treatment with medication for pain relief. The aim is to influence pain and muscle tension so that physical treatment, followed by more active movement, becomes possible. Together, we decide which steps and sequence suit your findings.

How we assess a joint that feels ‘blocked’

If a joint or part of your spine feels blocked, there can be different reasons. Mobility, muscle tension and pain influence one another. An examination helps clarify which structures are involved and whether manual treatment or another approach might be appropriate. Evocell can be included in a treatment plan with the aim of relaxing muscles. However, the couch does not realign vertebrae. Nor can a session reliably be expected to release a joint restriction. The method also does not reverse osteoarthritis, disc damage or narrowing around a nerve.

What is known about possible benefits

A controlled study of Evocell for non-specific back pain reported pain relief over a three-week course of treatment. This supports considering its use as an additional treatment component. The study was small and unblinded; the degree and duration of benefit vary between individuals and cannot be reliably predicted. We focus on a specific goal: less muscle tension and discomfort, so that straightening up, walking or subsequent exercises become easier. We discuss how you can recognise a helpful change. The extent and repetition of treatment depend on your findings, how you tolerate it and the progress observed.

How it fits with movement and other treatments

For many persistent back symptoms, appropriate active treatment is part of the plan. Exercises, everyday movement and a gradually adjusted level of activity can serve different purposes. Passive treatment on the couch does not replace these steps. Evocell is therefore mainly used during an acute episode of pain, with the aim of enabling you to move actively again afterwards with less discomfort. Depending on the findings, Evocell may be coordinated with manual treatment or physiotherapy. The addition should have an understandable goal, such as reducing muscle tension before subsequent movement. Several methods are not automatically better than a clearly justified treatment plan. We discuss what each step is intended to contribute and how we will assess its benefit together.

When Evocell is unsuitable or requires particular assessment

The instructions for use list pregnancy, acute blockage of a blood vessel, an aneurysm, recent surgery and certain implanted pacemakers or electrodes among the reasons not to use the device. Please tell us about such circumstances beforehand. Fractures that have not fully healed, recent implants, acute infections and various heart, circulatory or other conditions require particular medical assessment. This list does not replace a discussion of your health. In particular, if osteoporosis puts your bones at risk, new back pain first needs to be assessed for a possible fracture.

What to expect and next steps

Procedure and what to do afterwards

Before starting, we discuss your symptoms, relevant previous conditions and the planned treatment. You are positioned appropriately and should tell the team if the treatment is uncomfortable or new symptoms occur. Duration and further appointments are agreed according to the treatment plan. After treatment, take your time sitting up. If you feel dizzy or unsteady, tell us immediately. Please do not stand up on your own. Temporary muscle soreness and dizziness are among the possible side effects described. How you can be active afterwards depends on the findings and how you feel.

What matters for your decision

Treatment decision and costs

If you would like to clarify whether Evocell could usefully complement treatment for your back pain, tell us about your concern. Bring existing findings and information about previous treatments. We assess a recommendation from another practice in the context of your current situation. Before an agreed treatment, we discuss the scope of the service and the expected costs. If you have German statutory health insurance and would like to pay yourself for a service that is not covered, you receive a cost estimate beforehand. Before the service is provided, you sign to confirm in writing that you wish to have the treatment and agree to pay for it. If we have already recommended Evocell, arrange your next appointment through the route discussed with you.

Frequently asked questions

Is Evocell the same as shockwave therapy?
Evocell transfers mechanical impulses to the body through a couch. Targeted radial or focused shockwave therapy for individual areas is applied differently and has its own uses. Statements about the effectiveness of one method cannot automatically be transferred to the other.
Can Evocell release a sacroiliac joint restriction?
A feeling that a joint is ‘blocked’ needs assessment first. Evocell may be considered to support muscle relaxation. Reliably relieving a joint restriction is an aim of treatment, but cannot be promised.
Does this treat osteoporosis?
No. It does not replace osteoporosis investigations or treatment specifically for bone health. With osteoporosis and new back pain, a possible vertebral fracture must be considered in particular before a suitable measure is chosen.
Do I also need exercises?
The appropriate active treatment depends on your symptoms. For persistent back symptoms, a passive measure should support suitable movement and exercise. It does not replace them.
How many sessions are needed?
This cannot be decided in advance for every patient. The treatment goal, tolerability and progress determine whether further sessions appear worthwhile.
What should I report immediately during treatment?
Tell the practice team immediately about pain, dizziness, feeling unwell or other changes. You do not have to endure an uncomfortable treatment. Help with sitting up and standing is also available if needed.
